Got a new laptop a couple of weeks ago and I can finally start playing again on something decent. (My previous system was an old 2004 Dell with Intel integrated and an GeForce MX440 AGP8x, and I gave up trying with maybe 5 hours of gameplay left.) Anyway, my system is a Lenovo Y480, with Intel HD Graphics 4000 on the processor and NVIDIA GeForce GT 450M LE tied to the Intel graphics through NVIDIA Optimus. The game runs with nice performance, but the first thing I noticed was running on the NVIDIA GPU with VSync on produces a tear with a consistent height about the size of a Windows title bar on the top of the screen. This happens only on the NVIDIA GPU, and not the Intel one. The Intel one just produces uniform crappiness (i.e. low performance, because it's Intel). With VSync off, I get general jitteryness. Tried to disable composition and theming, but that didn't work, so I'm inclined to think it isn't Windows compositing related. I haven't really played with the settings inside NVIDIA Control Panel, because I'm not sure which ones I should adjust. My driver version is 306.23. Same happens for False.Genesis' update pack.

Did you try running on Linux? Easiest thing is to get a recent LiveCD (Mageia seems to be a good choice), slap gfx drivers on it, and try that.
Personally, for all windows versions after XP I've learned to blame windows first, not the software running on it.
Looks like a driver problem though.
(No problems here on notebook/desktop/anywhere...)

I've got a copy of Ubuntu. The unfortunate thing is Optimus, where the setup is not trivial. Can't disable integrated graphics through BIOS either. Grr...
