Title: Where is the fish form?

People talk about a fish form but none have said its location. Where exactly is it since its one of the two forms I'm missing.

It's in the Kelp Forest, inside a cave with a fairly obvious name. There's a puzzle inside, and once you've figured it out you will be granted the form.

Thanks for the info. Also where is the singing octopus I keep reading about.

The octopus is actually inside of another, much larger creature. Getting inside, though, is another story.

Try to snoop around dark areas you couldn't when you didn't have a light source like (I hope) you do now, and you'll find a picture of said large creature with a song to sing to it.
